This year, due to concerns about the COVID-19 delta variant, we are asking attendees to register in advance for our Rosh Hashanah family service. This family service will be held outside at Temple.

By registering, you attest that:

1. Everyone in your party, of the age of 12 or older, is currently vaccinated.

2. Everyone in your party, over the age of 2, will remain masked throughout the service.

In the "Notes" field below, please also list how many individuals over 12 and under 12 years of age will be in your party.
